A Premise History contains critical information that you have provided us about the individual who lives alone and has a significant medical condition. We will supply you with a form that will ask about the individual’s medical condition, doctors, hospital preferences, medical alarms, family contact phone numbers and more.
This information is entered into the 911 computer system for the involved address. If a 911 call is received from a “Premise History” address, we will be immediately notified of the information that has been provided to us.
